Medication

Indiana State Law requires written permission to administer any medications at school (prescription and over-the-counter, including cough drops). All medications must be kept in the nurse's office. The Medication Administration permission form may be found on EVSC's website or provided at school upon request.
 

Nurse Visits

When a student would like to visit the nurse, their teacher will fill out a nurse pass and send it with the student to the nurse's office. This allows the nurse to know the issue and that the teacher knows where the student is at all times. Students are sent to the nurse due to various issues—illnesses, injuries, medication administration, etc. There may be times when the problem warrants a call to the parent for notification purposes or for pick-up. You can always call the nurse's office with questions or concerns. 

Vision & Hearing

The nurse's office will also conduct vision and hearing screenings in the fall. If your student is experiencing difficulty with vision or hearing, you will be notified with a paper referral that will go home in the student's backpack. 
Immunizations are due on the first day of school for all kindergarteners. If the school does not receive this document on the first day, a 20-day waiver will automatically be issued. This will give time to get this information to the nurse or to provide the nurse with an upcoming appointment date and time to avoid exclusion. 


If your student has asthma, food allergies, diabetes, or other medical condition, a care plan form must be filled out at the beginning of each new school year and turned in to the nurse's office. This information will allow the nurse to familiarize the student's health needs and inform the teachers and cafeteria personnel of the concerns. When registering your student online, please use the Student Health Form to indicate health issues, 
both current and past history.
